Severe Headache and Blurred Vision in Pregnancy

Severe headache with blurred vision in pregnancy needs prompt medical assessment, especially if it is new, persistent, or accompanied by other warning signs. Contact your maternity team or an urgent medical service now for advice rather than waiting for a routine appointment.

Headache and visual changes can occur for less serious reasons, such as dehydration, lack of sleep, migraine, or changes in eyesight. During pregnancy, however, the combination can also be a warning sign of pre-eclampsia, a condition involving high blood pressure and effects on organs. Pre-eclampsia may occur during pregnancy or after birth, so these symptoms still matter in the postnatal period. A clinician may check your blood pressure, urine, symptoms, and blood tests to decide what is happening.

Stop driving if your vision is affected. Sit or lie somewhere safe, ask another adult to stay with you if possible, and contact your maternity unit, obstetrician, or urgent care service. Tell them how far along you are, when the headache began, whether the vision change is in one or both eyes, and whether you have taken any medicine. Do not take a new medicine or repeat a dose without checking that it is suitable in pregnancy.

Seek emergency help immediately if the headache is sudden or extremely intense, the vision loss or blurring is severe, or you also have weakness, confusion, difficulty speaking, fainting, a seizure, chest pain, or trouble breathing. Urgent assessment is also needed with pain under the ribs, repeated vomiting, sudden swelling of the face or hands, or reduced movement of the baby. Do not wait for the symptoms to settle if you feel seriously unwell.

Ask what your blood pressure and urine checks show, whether you need blood tests or monitoring, and which symptoms should prompt you to return. You can also ask whether the visual symptoms fit migraine or another eye problem, which pain relief is appropriate for you, and whether follow-up is needed after delivery.
