Severe Diaper Rash from Diarrhea: What to Know

Loose stools can soil the skin repeatedly and keep it damp. Digestive contents and frequent wiping may then strip away the barrier, leaving redness, tenderness, raw areas, and cracks. The rash may worsen while diarrhoea continues. Antibiotics, a change in diet, infection, or another illness may contribute to diarrhoea, so the skin problem should be considered alongside the child’s general condition.

Check the nappy often and change it promptly. Rinse stool away with lukewarm water, use a soft cloth, and pat dry. Apply a generous layer of fragrance-free barrier ointment after each change; do not scrub away every trace of the previous layer if the skin is clean. Give brief air time when possible and avoid perfumed wipes, talc, and household remedies. Keep offering the child’s usual fluids or feeds. Contact a clinician promptly if the rash is raw, bleeding, blistered, oozing, rapidly spreading, or very painful. Seek urgent care if the child has fever, repeated vomiting, blood in the stool, a dry mouth, markedly fewer wet nappies, unusual drowsiness, or difficulty drinking. These features can signal an illness or dehydration that needs assessment, not just a skin cream.

Ask what may be causing the diarrhoea, how to protect broken skin, whether a yeast rash or bacterial infection is present, and which products should be stopped. Tell the clinician when the diarrhoea began, how often stools occur, whether fluids are staying down, and what medicines have been given.
