What should you know about seronegative rheumatoid arthritis symptoms?

Seronegative rheumatoid arthritis can cause inflammatory pain, swelling, and stiffness even when rheumatoid blood markers are negative. Symptoms often involve the hands, wrists, feet, or other joints and may develop gradually. Blood results are only one part of assessment; the examination and symptom pattern also matter.

Rheumatoid arthritis may present with hand swelling, finger stiffness, joint pain, joint swelling, and morning joint stiffness. Stiffness after waking or resting can make gripping, dressing, or opening containers difficult. Symptoms may occur on both sides, although patterns vary. Warmth, reduced movement, tiredness, or a general unwell feeling can accompany active inflammation.

A negative rheumatoid factor or related antibody test does not remove the need to assess persistent inflammatory symptoms. Other causes of joint symptoms can look similar, so diagnosis should not be made from one symptom or one laboratory result.

Keep a brief record of swollen joints, stiffness after waking, pain at rest, fatigue, and changes in hand function. Photograph visible swelling when it appears, and bring a list of medicines and recent illnesses. A clinician may examine several joints, request blood tests, or use imaging to look for inflammation. Anti-inflammatory medication may relieve symptoms for some people, but it should be chosen with attention to other health conditions and medicines.

Arrange medical review when swelling or stiffness persists, returns repeatedly, or limits normal hand use. Seek urgent care for a hot, very painful joint with fever, sudden loss of movement, or severe illness. New weakness, persistent numbness, or a hand that becomes cold or discoloured also warrants prompt assessment.

Ask which features suggest inflammatory arthritis, what other conditions are being considered, and whether imaging is appropriate. Discuss how symptoms will be monitored when blood markers are negative, which activities are safe, and when treatment should be reviewed.
