What should you know about septal panniculitis erythema nodosum?

Panniculitis means inflammation in the fatty layer beneath the skin. In erythema nodosum, painful, firm lumps often appear on the fronts of the lower legs. They may look red or warm at first and later fade through bruise-like colour changes without opening into ulcers.

The skin finding is a reaction pattern rather than a single cause. A recent infection, a new medicine, or an inflammatory condition can be relevant, and some episodes have no clear trigger. Joint aches, ankle discomfort, tiredness, or fever can occur alongside the nodules.

Book a clinical assessment for new painful shin lumps, particularly when they are accompanied by fever, joint symptoms, breathing complaints, bowel symptoms, or a recent illness. The clinician may examine the skin, review medicines and recent infections, and decide whether tests or a biopsy are needed.

Resting the legs, elevating them, and using simple comfort measures recommended by a clinician may ease tenderness. Do not start oral corticosteroid treatment without advice, because steroids can worsen some untreated infections and require attention to the underlying cause.

Seek prompt care if nodules are rapidly multiplying, pain is severe, you feel feverish or faint, or the skin becomes ulcerated or unusually dark. Get urgent help for trouble breathing, chest pain, facial swelling, or sudden severe illness. These features may point beyond an uncomplicated skin reaction.

Ask what causes should be considered, whether your medicines or recent infections are relevant, whether you need tests, and how long the nodules should take to settle. Ask whether anti-inflammatory treatment is suitable and what changes should trigger review.
