What should you know about selective IUGR in twin pregnancy?

Twin size differences are judged using each baby's growth pattern, the placenta or placentas, amniotic fluid, and blood flow. A smaller twin may be affected by placental function, while some size difference can occur without evidence that either baby is unwell.

The type of twins matters. When twins share a placenta, blood vessels and shared circulation can make assessment more complex. Your specialist will explain whether the twins share placental or amniotic spaces and how that changes surveillance.

Attend the recommended Growth Scan appointments so measurements, fluid, and blood flow can be compared over time. Fetal Heart Monitoring may be used to check each twin's heart pattern and response as pregnancy advances.

If findings become concerning, your team may recommend assessment in hospital or plan birth. Induction of Labour is considered only when the pregnancy and both babies are suitable for that approach; a caesarean or another plan may be advised depending on presentation and wellbeing.

Seek prompt advice for a clear reduction in either twin's movements, vaginal bleeding, leaking fluid, contractions, severe abdominal pain, or symptoms such as severe headache and visual disturbance. Use the movement guidance given by your maternity team because twins can have different patterns.

Ask whether the twins share a placenta, how the growth difference is changing, which Doppler or heart-monitor findings are concerning, and what birth plan would protect both babies if the results change.
