Scrotal cellulitis and antibiotic treatment

Cellulitis is an infection of the skin and tissues beneath it. On the scrotum, it may cause warmth, redness, tenderness, swelling, or skin that feels tight. A small cut, insect bite, shaving injury, ulcer, or another break in the skin can allow bacteria to enter. The appearance alone cannot show how deep an infection extends.

A clinician must also consider other causes of scrotal pain or swelling, including inflammation around the testicle, a hernia, an abscess, or a rapidly progressive deep soft-tissue infection. Antibiotics treat susceptible bacterial infection; they do not treat every cause of scrotal swelling.

Seek same-day medical assessment rather than self-treating a swollen or painful scrotum. The clinician may examine the skin, testicles, groin, and nearby lymph nodes, then choose an antibiotic based on the suspected bacteria, allergy history, other medicines, and local clinical factors. Take the prescribed course exactly as directed and do not use leftover antibiotics. Keep the area clean and dry, avoid squeezing any lump, and wear supportive comfortable underwear if that reduces movement. Do not apply harsh antiseptics unless advised.

Go to emergency care for sudden or severe testicular pain, rapidly spreading redness or swelling, skin turning dark or blistering, fever with shaking chills, vomiting, faintness, confusion, or a very unwell feeling. These features can signal a time-sensitive problem that antibiotics alone may not address. If symptoms are mild but do not improve after treatment begins, contact the prescriber promptly rather than changing the medicine yourself.
