Scarlet fever without a sore throat

Scarlet fever is linked to infection with group A Streptococcus, the same group of bacteria associated with Strep Throat. Throat pain may be mild, absent, or difficult for a young child to describe. Look for a fine, rough-feeling rash, flushed cheeks, a pale area around the mouth, fever, headache, nausea, or tender neck glands. A clinician may examine the skin and throat and decide whether testing is needed; a rash alone cannot confirm the cause.

Arrange a clinical review if a new sandpaper-like rash appears with fever or if the person has contact with someone who has a confirmed streptococcal infection. Record when the fever and rash began, whether the rash blanches when pressed, and any medicines already taken. Offer frequent fluids and comfortable clothing. Do not share cups, towels, or utensils, and wash hands after coughing, sneezing, or touching nasal secretions. If prescribed, take Antibiotic Treatment exactly as directed and complete the course unless the prescriber changes it.

Seek urgent help for trouble breathing, blue or grey lips, severe drowsiness, confusion, a stiff neck, repeated vomiting, or inability to drink. Get prompt medical advice if the rash is rapidly spreading, painful, blistering, or does not fade when pressed, or if fever persists or the person is worsening. Babies, people with weakened immunity, and those with significant medical conditions need a lower threshold for review.
