Scarlet fever rash stages

The illness often starts with throat symptoms and fever, followed by a fine, sandpaper-like rash that may begin on the trunk or in skin folds before spreading. The skin can look flushed, while the area around the mouth may remain paler. As the illness settles, the rash usually loses intensity; peeling may then affect the fingers, toes, or other areas. These stages are clues rather than a diagnostic test. During the active rash, use lukewarm water, soft clothing, and a fragrance-free moisturiser for dry skin. Keep nails short and avoid scratching or rubbing. During peeling, leave skin attached, moisturise intact skin, and protect hands and feet from friction. Offer drinks and rest. Contact a clinician when the rash is new, spreading, unusually painful, or not following an improving course, particularly with [Fever](/symptoms/fever/). Get urgent help for breathing or swallowing difficulty, confusion, severe drowsiness, dehydration, purple spots, extensive blistering, or rapidly worsening swelling. Do not wait for a later peeling stage to obtain an assessment, and do not use the apparent stage to decide whether treatment is needed.
