Scarlet fever rash on the feet

Scarlet fever produces a fine, rough rash that can extend over several body areas, including the legs and feet. The feet may feel itchy, warm, tender, or dry, and peeling can appear as the skin recovers. A foot rash has many possible causes, so its location does not establish scarlet fever. A clinician will consider it with a sore throat, fever, tongue changes, or swollen neck glands. Wash gently with lukewarm water, dry carefully between the toes, and use a plain moisturiser on dry areas that are not broken or weeping. Choose breathable socks and roomy footwear; avoid heat, friction, and perfumed products. Do not pull away peeling skin or share socks and towels. Encourage fluids and rest while the person is unwell. Arrange medical assessment rather than treating the foot rash alone. Seek prompt advice if the feet become increasingly painful, swollen, hot, cracked, or ooze fluid, or if walking becomes difficult. A child with a new foot rash and sore throat or fever should be assessed even if the rash looks mild. Do not infer the diagnosis from the feet alone.
