How long does a scarlet fever rash last?

Scarlet fever can begin with a fine, rough rash during an illness that may also include sore throat and fever. The colour and texture should gradually settle as the underlying infection improves. Skin can peel afterward, particularly around the hands and feet; this does not by itself mean the illness is worsening. A rash that persists, changes character, or appears without the expected throat symptoms needs assessment because other conditions can look similar. Follow the treatment plan given by the clinician and complete prescribed [Antibiotic Treatment](/treatments/medical-management/antibiotic-treatment/) unless the prescriber changes it. Offer regular drinks and rest. Keep the skin clean, use lukewarm water, pat rather than rub, and apply a simple fragrance-free moisturiser to dry or peeling areas. Avoid picking flakes and sharing towels, clothes, cups, or cutlery while the person is unwell. Mark the day the rash appeared and note whether fever, throat pain, or itching is improving; this gives the clinician a clearer picture.

Contact a clinician if the rash is not beginning to settle as the child or adult otherwise recovers, returns after fading, or is joined by a new fever or worsening throat pain. Seek urgent help for breathing or swallowing difficulty, severe dehydration, confusion, marked drowsiness, rapidly spreading blistering, or purple spots that do not fade with pressure. Timing alone cannot diagnose scarlet fever, so do not wait for the rash to disappear before arranging an assessment.
