How scarlet fever rash may look on dark skin

Scarlet fever can cause a fine, rough rash that often starts on the chest or stomach and may spread. On darker skin, colour changes may be less obvious or look brown, purple, grey, or deeper than the surrounding skin. Run clean fingertips gently over the skin without scratching: a sandpaper-like feel, warmth, dryness, or later peeling can be useful clues. Fever, sore throat, a coated tongue, and swollen neck glands may appear with the skin changes. A photograph in natural light can help a clinician compare changes over time.

Arrange a clinical assessment when a rough new rash occurs with fever or sore throat, particularly if it is spreading. Mention the person’s usual skin tone and describe changes in texture, warmth and sensation, not only colour. Keep the skin cool, use a gentle moisturiser if it becomes dry, and avoid fragranced products or vigorous rubbing. If scarlet fever is confirmed, follow the prescribed treatment and keep hands, towels and drinking items separate until the clinician says normal contact is appropriate.

Seek urgent help for breathing difficulty, swelling of the lips or tongue, faintness, confusion, a rapidly spreading purple or bruise-like rash, skin blisters, severe pain, or peeling with mouth or eye sores. Get prompt advice if fever persists, swallowing becomes difficult, urine becomes very dark, or the person is drinking much less. A rash can be difficult to identify visually, so worsening symptoms deserve examination regardless of skin tone.
