Scarlet fever and puffy eyes: what to know

Scarlet fever is linked to a toxin-producing streptococcal throat infection and commonly includes a rough-feeling rash, sore throat and fever. Eyelid swelling can have other explanations, including irritation, allergy, rubbing, lack of sleep, or a separate infection. The pattern matters: swelling in one eye, redness of the eye surface, discharge, pain with eye movement, or changes in vision should not be attributed to scarlet fever without an examination. A clinician may check the throat, skin and eyes and decide whether testing for strep is appropriate.

Arrange medical advice if puffy eyes occur with a sore throat, fever, a fine rough rash, or tender neck glands. Keep the eye area clean, avoid rubbing, and do not share towels. Encourage fluids and rest while waiting for advice. If scarlet fever is diagnosed, take prescribed medicine exactly as directed; treatment addresses the underlying infection, while a cool clean cloth may soothe mild eyelid discomfort. Do not put non-prescribed drops, creams, or household products near the eye.

Seek urgent medical help for trouble breathing, marked facial swelling, severe eye pain, pain when moving the eye, a change in vision, inability to open the eye, confusion, or a rapidly worsening illness. Contact a clinician promptly when swelling persists, affects one side, or comes with fever and a painful throat. These features can point to an eye or facial problem that needs a different assessment.
