Scarlet fever on the tongue: what it can look like

A bright red tongue with prominent bumps is sometimes called a strawberry tongue. In scarlet fever, it can appear with a rough rash, sore throat, fever, flushed cheeks, or swollen glands. The tongue may look coated at first and become redder as the illness develops.

Tongue changes are not specific to one condition. Mouth irritation, other infections, medicines, and dehydration can alter the tongue, so a photograph or description alone cannot establish the cause. A clinician should consider the whole pattern and examine the throat and skin.

Arrange prompt medical assessment when tongue changes occur with a rough rash or sore throat. Offer cool fluids and soft foods if swallowing is comfortable, and avoid spicy or acidic foods if they sting. Do not scrape the tongue or share cups, cutlery, toothbrushes, or towels.

Wash hands after contact with saliva or nasal secretions. If the clinician recommends Antibiotic Treatment, follow the prescribed schedule and ask when close contact and school or work attendance are appropriate.

Seek urgent care if tongue or throat swelling affects breathing, swallowing, or speaking; if saliva cannot be swallowed; or if the person becomes very weak, confused, or dehydrated. Rapidly spreading rash, facial swelling, or severe worsening pain also needs immediate review.
