Scarlet fever without fever: what to know

Fever is one possible feature, not a requirement for recognising every case. Scarlet fever is associated with bacteria that also cause Strep Throat, and may produce a rough, sandpaper-like rash, sore throat, red tongue, flushed skin, or swollen glands. A recent dose of fever medicine, an inaccurate reading, or an early illness can make fever less obvious.

A rash alone has many possible causes, including viral illnesses and skin irritation. The pattern, throat symptoms, examination, and testing help a clinician decide what is happening; absence of fever does not confirm or rule out scarlet fever.

Contact a clinician promptly if the rash is rough, spreading, or paired with a sore throat, swollen glands, or a red tongue. Note when the rash began, where it started, any measured temperature, medicines already given, and whether the person is drinking and urinating normally.

Until assessed, encourage fluids, rest, and gentle skin care. Do not share towels, cups, or cutlery, and wash hands regularly. If antibiotic treatment is prescribed, give it exactly as directed and ask when the person may return to school, nursery, or work.

Get urgent help for trouble breathing or swallowing, severe weakness, confusion, signs of dehydration, rapidly spreading skin changes, or swelling of the lips or face. Seek prompt review if throat pain is severe, the person cannot drink, or symptoms worsen even though fever is absent.
