Scarlet fever without antibiotics: what to know

Scarlet fever is an illness linked to the same bacteria that cause Strep Throat. A fine, rough rash can occur with a sore throat, fever, flushed cheeks, or swollen glands, but symptoms vary. A clinician may assess the throat and arrange testing before deciding whether antibiotic treatment is appropriate.

Avoid assuming that a fading rash means the infection has fully settled. Untreated infection can remain contagious, and a child or adult may pass it to close contacts through coughs, sneezes, or shared items.

Arrange a same-day or prompt medical review if scarlet fever is suspected. Until advice is given, rest, drink fluids, use age-appropriate symptom relief as directed on the label, and avoid sharing cups, towels, or utensils. Wash hands after coughing, sneezing, or caring for the sick person.

If a clinician prescribes Antibiotic Treatment, follow the instructions and complete the course unless the prescriber changes it. Keep the person away from school, nursery, or work until a clinician confirms when returning is appropriate.

Seek urgent help if breathing becomes difficult, swallowing fluids is hard, the person is unusually drowsy, dehydration is developing, or a rapidly worsening rash is accompanied by severe illness. A persistent high fever, marked neck swelling, or severe throat pain also needs prompt assessment.

Ask whether testing is needed, whether the illness could be Strep Throat, how long the person should stay home, and which symptom medicines are suitable for their age and health conditions.
