Scarlet Fever in the Mouth: What to Know

The tongue may look coated at first and then become noticeably red with prominent bumps. The throat can look inflamed, and swallowing may hurt. These findings can occur when toxin-producing group A Streptococcus bacteria cause an infection in the throat; the mouth appearance alone cannot confirm the diagnosis. Scarlet fever is related to [Strep Throat](/conditions/strep-throat/), but other infections can produce similar mouth or throat changes.

Arrange a clinical assessment if a child or adult has a sore throat with fever, rash or a strikingly red tongue. A clinician may examine the throat and decide whether testing is appropriate. Offer frequent fluids, choose soft foods if swallowing hurts, and use age-appropriate medicine only as directed by a pharmacist or clinician. Handwashing, covering coughs and avoiding shared cups can reduce spread while symptoms are active.

Seek urgent medical help for trouble breathing, inability to swallow fluids, drooling, unusual drowsiness, signs of dehydration or rapidly worsening swelling. Contact a clinician promptly for persistent fever, worsening throat pain, a spreading rash or marked neck swelling. A red tongue by itself is not enough to identify scarlet fever, so worsening symptoms deserve reassessment.
