Scarlet fever symptoms in infants

Infants may not be able to describe throat pain or headache. Instead, they may cry during feeds, refuse liquids, drool more than usual, sleep differently, or seem unusually unsettled. Scarlet fever can also cause Fever, a rough rash, flushed cheeks, and swollen glands. The rash may be harder to recognise on darker skin, so changes in texture can be useful to notice. Arrange prompt medical advice for an infant with a fever and new rash, especially when feeding changes or throat symptoms are present. Offer breast or formula feeds frequently and monitor wet nappies. Do not give over-the-counter medicines unless a healthcare professional confirms the product and dose are suitable for the infant. Keep the infant away from shared cups, utensils, and close contact with vulnerable people until a clinician explains the infection and treatment plan.

Seek emergency help if the infant has breathing difficulty, blue or grey lips, a seizure, a stiff neck, marked limpness, unusual difficulty waking, or cannot keep fluids down. Seek urgent assessment for fewer wet nappies, a dry mouth, persistent vomiting, a rapidly spreading or bruised-looking rash, or fever in a very young infant. A Sore Throat may be difficult to identify at this age, so feeding and behaviour matter.

Ask whether the infant needs an examination or throat test, how to maintain feeds, which treatment is suitable, and how to measure improvement. Confirm which temperature or hydration changes require immediate help and when close contacts should be informed.
