Scarlet fever and eye infection: what to know

Scarlet fever is a bacterial illness linked with a sore throat, fever and a fine rough rash. Eye redness, sticky discharge, pain, light sensitivity or blurred vision is not a defining feature. It may be irritation, conjunctivitis, another infection, or a problem affecting the eye itself. The presence of an eye symptom cannot confirm scarlet fever, and an eye infection does not by itself point to scarlet fever. A clinician may examine the throat, skin and eyes and decide whether a throat swab or eye assessment is needed.

Arrange a clinical review if a child or adult has a suspected scarlet-fever illness together with a red or uncomfortable eye. Wash hands often, avoid rubbing the eye, and do not share towels, pillows or face cloths. Clean discharge gently with fresh cotton and clean water, using a new piece for each wipe. Do not use leftover eye drops or someone else’s medicine. Seek urgent medical help for severe eye pain, reduced or blurred vision, marked light sensitivity, swelling around the eye, inability to open the eye, or a child who seems unusually drowsy or very unwell. Same-day advice is sensible when fever and a spreading rough rash occur with eye discharge, worsening throat pain, difficulty drinking, or reduced urination. Emergency help is needed for breathing difficulty, inability to swallow saliva, confusion, or rapid deterioration.

Ask whether the eye problem needs a separate diagnosis, whether a throat test is appropriate, which symptoms should trigger urgent review, and how to reduce spread at home or school. Mention contact-lens use, recent medicines, allergies, the time the eye symptoms began, and whether one or both eyes are affected.
