Scarlet fever and cold hands and feet

Hands and feet can feel cool during a fever or when the body is responding to illness. Scarlet fever more typically involves a sore throat, fever, a fine rough rash, and sometimes swollen glands. A rash or sore throat supports the need for clinical assessment, but cold extremities alone do not identify the cause. Persistent coolness between illnesses may relate to anaemia, thyroid underactivity, reduced blood flow, or simple sensitivity to cold. Look at the child’s overall colour, alertness, breathing, and ability to drink rather than judging severity from hand temperature alone.

Keep the child comfortably warm with ordinary clothing, offer frequent fluids, and check their temperature if they feel unwell. Do not use very hot water or direct heat on numb fingers or toes. Arrange medical advice if scarlet fever is suspected, particularly when fever and sore throat occur with a rough rash. A clinician may examine the throat and skin and decide whether testing or antibiotic treatment is appropriate. Record when the coldness began and whether it improves as the fever settles.

Seek urgent help if the child is difficult to wake, has trouble breathing, develops blue, grey, or blotchy skin, feels unusually cold with marked weakness, or cannot keep fluids down. Get prompt advice if one limb is much colder or changes colour, or if severe pain, swelling, or numbness develops.
