Scarlet fever and bloodshot eyes: what to know

Scarlet fever is usually recognised through a throat illness, fever and a fine rough rash rather than bloodshot eyes. Redness can come from irritation, dryness, allergy or Conjunctivitis, which may also cause discharge, itching or eyelid crusting. Allergic Conjunctivitis often feels itchy and may affect both eyes, while infectious conjunctivitis can spread between people. Pain, light sensitivity or blurred vision changes the level of concern. Eye redness occurring during another illness should therefore be described clearly rather than assumed to be part of scarlet fever.

Arrange medical advice if bloodshot eyes occur with fever, a new rash, sore throat or unusual discharge. Avoid rubbing your eyes, wash hands before and after touching the face, and do not share towels, pillows or eye cosmetics. Remove contact lenses until an eye professional says they are safe to wear again. Do not use someone else’s drops, particularly leftover antibiotic or steroid drops. A clinician can examine the eye and decide whether treatment or an eye review is needed.

Seek urgent eye care for severe eye pain, reduced or blurred vision, strong light sensitivity, injury, a chemical splash, marked swelling or difficulty moving the eye. Prompt assessment is also needed when redness is rapidly worsening or accompanied by severe headache, vomiting or a very unwell appearance. Rinse a chemical exposure with clean running water while arranging emergency help.

Ask whether the eye findings fit irritation, allergy or conjunctivitis, whether the throat and rash suggest a separate infection, and which drops or hygiene measures are appropriate. Mention contact lens use, discharge, pain, vision changes and recent exposure to anyone with red eyes.
