Scarlet fever and sore throat: what to know

A sore throat is a common presentation of scarlet fever. The illness is linked to the same group of bacteria that can cause Strep Throat, and the throat may look red with painful swallowing. A fine, sandpaper-like rash can begin on the chest or neck and spread, while the cheeks may look flushed. Swollen glands at the front of the neck can accompany the throat infection. Symptoms overlap with viral illnesses, so the rash and examination matter when a clinician assesses the cause.

Arrange a clinical assessment if a sore throat is accompanied by a rough rash, fever or a bright red tongue. A clinician may examine the throat and decide whether testing or prescription treatment is appropriate. Rest, drink fluids in small regular amounts, and choose soft foods if swallowing hurts. Do not share cups, utensils or towels, and wash hands after coughing or sneezing. If Antibiotic Treatment is prescribed, follow the directions and complete the course unless the prescriber changes it.

Seek urgent help for trouble breathing, inability to swallow saliva, marked drowsiness, severe dehydration or rapidly worsening symptoms. Get prompt medical advice if the fever persists, the rash is spreading quickly, the throat pain is strongly one-sided, or the neck becomes very swollen. A sore throat with a rash deserves assessment because several infections can look alike and treatment decisions depend on the examination.

Ask whether the throat findings and rash fit scarlet fever, whether a swab is useful, how to ease swallowing, and when it is reasonable to return to school or work. Mention any medicine allergy, recent antibiotics and contact with someone who had a similar illness.
