What should you know about sarcomatoid adrenocortical carcinoma?

The adrenal cortex is the outer part of an adrenal gland, which sits above each kidney. Adrenocortical carcinoma starts in cells of that outer layer. Sarcomatoid describes an area whose cells have a spindle-shaped or sarcoma-like appearance under the microscope. The diagnosis relies on tissue examination, immunostains, imaging, and the pathologist's assessment of the tumour's different components.

Some adrenal tumours make hormones and can cause changes such as unusual weight change, high blood pressure, weakness, or excess hair. Others do not produce noticeable hormones and may present through abdominal or back discomfort or a mass found during imaging. Symptoms do not establish the diagnosis; several non-cancerous conditions can cause similar changes.

Evaluation commonly includes hormone testing, cross-sectional imaging, and careful pathology review. Ask whether the tumour is confined to the adrenal gland, whether nearby or distant areas need assessment, and whether a specialist centre should review the slides. When removal is feasible, tumour removal surgery may be discussed. Chemotherapy or other systemic treatment can be considered according to pathology, spread, health, and treatment goals.

Keep a record of new symptoms, medicines, blood-pressure readings if requested, and scan appointments. A coordinated team can explain how hormone control, surgery, and cancer treatment fit together.

Get urgent medical help for severe abdominal pain, fainting, confusion, marked weakness, chest pain, or severe breathlessness. Contact your care team promptly for persistent vomiting, rapidly worsening swelling, a major change in blood pressure, or symptoms suggesting a hormone disturbance. If you are taking steroid replacement, do not stop it suddenly; ask the prescribing team what to do during illness.

What did the pathology review show? Is the tumour producing hormones? Has staging been completed? Can it be removed? What are the goals and expected effects of surgery or systemic treatment? Should the pathology be reviewed by an endocrine oncology specialist?
