Sarcoidosis granulomas on skin: what should you know?

A granuloma is a compact collection of immune cells formed in response to ongoing inflammation. In cutaneous sarcoidosis, granulomas sit within the skin and may produce firm, smooth papules, nodules, or plaques. They can be skin-coloured, red-brown, or purple, and some people have little discomfort while others notice tenderness, itch, or sensitivity.

The same word does not identify the cause by itself. Infections, foreign material, and other inflammatory conditions can also produce granulomas. A dermatologist may recommend a biopsy, with other tests chosen according to the history and examination. Skin findings can occur with sarcoidosis elsewhere in the body, so symptoms such as persistent cough, breathlessness, eye irritation, or joint pain should be mentioned.

Keep the area clean, avoid picking at it, and record changes in size, colour, tenderness, or number. Take photographs in consistent lighting and bring your current medicine list to the appointment. Treatment depends on how active and extensive the skin disease is, whether it affects function, and whether other organs are involved.

For a limited lesion, a clinician may discuss a steroid injection into the skin. More widespread or troublesome disease may lead to discussion of oral corticosteroid treatment or another prescribed approach. These medicines should be selected and monitored by a clinician because the benefits and side effects depend on the individual situation.

Seek urgent care for new breathing difficulty, chest pain, severe eye pain, sudden vision change, or marked facial swelling. Contact a clinician soon if lesions ulcerate, grow quickly, become hot and painful, or appear with fever. A stable, symptom-free lesion still merits planned assessment when its cause is unknown.

Do these findings need a biopsy? Could infection or another granulomatous condition look similar? Should I have an eye, lung, or joint review? Would a local injection or oral treatment be appropriate, and how would it be monitored?
