Sarcoidosis facial rash: what should you know?

Skin sarcoidosis happens when clusters of inflammatory cells called granulomas form in the skin. On the face, the changes may look like red-brown, violaceous, or skin-coloured bumps, smooth plaques, or areas of altered texture. The surface may be fairly smooth rather than flaky, and lesions can be scattered around the nose, cheeks, eyelids, or ears.

These appearances overlap with many other skin conditions. Atopic dermatitis and contact dermatitis often cause itch or irritation, while psoriasis may produce more defined scaly plaques. Drug rashes, heat rash, ringworm, scabies, and intertrigo have different patterns and triggers. A facial rash is therefore a reason for assessment, not proof of sarcoidosis.

Arrange a dermatology review if the rash persists, returns, or changes. Bring a list of medicines and supplements, note when the facial changes began, and take clear photographs if the appearance varies. The clinician may examine your skin and ask about breathing symptoms, eye symptoms, joint discomfort, fatigue, or other changes because sarcoidosis can affect more than skin.

Do not squeeze or repeatedly scrub the lesions. Use a gentle cleanser and a bland moisturiser, and protect exposed skin from strong sunlight. Avoid starting steroid creams on the face without advice, since facial skin can be sensitive and treatment depends on the diagnosis.

Seek prompt medical care for a rash with eye pain, light sensitivity, blurred vision, facial weakness, breathing difficulty, chest pain, or rapidly increasing swelling. Arrange routine assessment for a persistent facial rash, especially if it leaves changes in colour or texture, involves the eyelids, or appears alongside unexplained fatigue or joint symptoms.
