Sandpaper rash on the chest: what to know

The chest is exposed to sweat, clothing friction, sunscreen, fragrances, jewellery, and hair or skin products. Heat rash may appear where warmth and moisture are trapped. Contact dermatitis can follow a new detergent, fabric, adhesive, or cosmetic. Atopic dermatitis tends to be dry and itchy, while psoriasis may form thicker scaly plaques. Ringworm can enlarge gradually, and intertrigo is more likely where skin surfaces rub together. Describe the texture, colour changes, border, and distribution rather than relying on the word rash.

Keep the chest cool and dry, wear loose breathable clothing, and pause newly introduced fragranced products. Wash gently without a loofah or scrub, pat dry, and use a plain moisturiser if the skin is dry. Do not share towels, and avoid applying multiple medicated creams at once because they can obscure the pattern. Mark the edge or take a dated photograph to notice spread. A clinician can select treatment after examining the surface and asking about exposures.

Arrange prompt assessment for a rash that spreads quickly, becomes painful or warm, drains fluid, blisters, peels, or appears with fever. Seek urgent help for breathing trouble, facial swelling, faintness, or a rapidly worsening illness. A persistent or recurrent chest rash should be reviewed, particularly if it follows a medicine or affects the nipples, skin folds, or mucous membranes.

Ask whether sweat, friction, a product, eczema, or infection best fits the pattern; whether a swab or other test is needed; and what can safely be applied to the chest.
