Sandpaper rash in an infant: what to know

Infant skin can feel rough when it is dry, inflamed, blocked by heat, or reacting to a product. Atopic dermatitis often causes itchy, dry patches; contact dermatitis may follow exposure to wipes, detergent, fabric, or cream. Heat rash tends to occur where sweat is trapped. Scabies can be intensely itchy and may affect more than one household member. Ringworm often has a more defined edge. These patterns overlap, and a baby’s age, temperature, feeding, medicines, and general behaviour help guide assessment.

Keep the infant cool, use loose cotton clothing, and bathe with lukewarm water and a fragrance-free cleanser. Pat rather than rub, then apply a plain moisturiser while the skin is still slightly damp. Stop any newly introduced product until the cause is clearer. Do not use adult acne products, essential oils, or steroid creams unless a clinician has advised them. Take a clear photo and note when the rash began, where it started, and whether itching or fever followed.

Contact a clinician promptly for a baby with fever, poor feeding, unusual sleepiness, breathing difficulty, facial swelling, rapidly spreading rash, blisters, purple or bruise-like marks, skin peeling, or a rash involving the eyes or mouth. Seek urgent help if the infant seems seriously unwell. Even without these signs, arrange an examination when the rash persists, recurs, or is difficult to soothe.

Ask what features distinguish eczema, heat rash, allergy, and infection in your infant, which moisturiser is suitable, and whether siblings or caregivers need checking. Mention every medicine, detergent, wipe, and skin product used recently.
