What should you know about salvage radiotherapy for prostate cancer?

Salvage radiotherapy treats tissues near the prostate after surgery when prostate cancer may remain or may have returned. The radiation team uses pathology, PSA changes, examinations, and scans to decide whether the prostate bed, lymph nodes, or another area should be targeted. The recommendation is individual and can include hormone treatment as well.

Radiotherapy is planned with imaging and delivered from outside the body over a course set by the oncology team. It is different from brachytherapy, which places a radiation source in or near a treatment area, and from targeted radioisotope therapy, which carries radiation through the bloodstream to selected cells. The suitable approach depends on where disease is suspected and on previous treatment.

Because the prostate has been removed, nearby bladder, bowel, and sexual-function tissues may receive some radiation. Possible effects include urinary urgency, bowel irritation, tiredness, and changes in erections. Your team can monitor and treat symptoms during follow-up.

Take your PSA timeline, operation report, pathology, imaging, and medicines to the consultation. Discuss urinary leakage, difficulty passing urine, bowel symptoms, and sexual health before planning starts. Ask how to prepare your bladder and bowel, whether hormone treatment is recommended, and how the team will protect nearby organs. Keep a record of new symptoms during the course.

Tell the radiation team quickly about inability to pass urine, severe pain, persistent vomiting, heavy bleeding, fever, or worsening bowel symptoms. Emergency care is needed for chest pain, severe breathing difficulty, fainting, or sudden weakness. A PSA change requires discussion with the oncology team; it does not by itself identify the location of cancer.
