What should you know about salvage radiation for prostate cancer?

Salvage radiation is radiotherapy given after prostate removal when tests suggest prostate cancer may remain or has returned. It is directed at the prostate bed and, when appropriate, nearby lymph-node areas. Doctors consider the PSA pattern, pathology, scans, urinary and bowel function, previous treatments, and general health before recommending it.

External-beam radiotherapy uses carefully planned radiation delivered during separate appointments. The team maps the treatment area with imaging and positions you so the beam reaches the intended tissues while limiting exposure to nearby organs. Some people are also offered hormone treatment, depending on the risk features and whether disease outside the prostate bed is suspected.

Common areas for discussion include urinary frequency, bowel changes, tiredness, and sexual function. Previous surgery can affect how symptoms develop. Difficulty passing urine, new bleeding, or worsening bowel symptoms should be reported rather than ignored.

Bring your PSA history, prostatectomy pathology, scan reports, and medication list to the radiation appointment. Ask whether the target is the prostate bed alone or includes lymph nodes, whether hormone treatment is advised, and how bowel and bladder preparation works. Tell the team about urinary leakage, bowel disease, implanted devices, and current sexual-function concerns so planning can account for them.

Contact the oncology team promptly for inability to pass urine, heavy rectal or urinary bleeding, fever, severe abdominal pain, or sudden worsening weakness. Seek emergency help for chest pain, severe breathlessness, fainting, or new one-sided weakness. During follow-up, a rising PSA or new bone pain needs review, but it does not by itself show where cancer is located.
