What the injury involves

The growth plate is the area of developing tissue near the end of a child’s bone. In a type five injury, force compresses this plate. The initial pain or swelling can resemble other injuries, and the compression may be difficult to see clearly on an early plain X-ray. The clinician may use the examination and additional imaging when needed.

Because the growth plate helps the bone lengthen, follow-up is part of care even when symptoms settle. A disturbance can later affect growth or the shape of the nearby joint. The child’s age, injured bone, alignment, circulation, examination findings and imaging all influence the plan.

What to do next

Keep the injured limb protected and follow instructions about weight-bearing, elevation and pain relief. Do not test the limb through sport or forceful stretching while the diagnosis is being clarified. Fracture casting may protect a stable injury, while a displaced or complicated injury may need fracture fixation surgery. Only the treating team can select the appropriate approach.

Keep every review appointment and ask when repeat imaging is needed. Return to school activities, sports and climbing should be gradual and clinician-guided. Tell the team about numbness, increasing tightness or pain that is not controlled by the recommended plan.

When to seek urgent care

Seek urgent help for pale, blue or cold fingers or toes, severe swelling, worsening pain, new numbness, inability to move the digits, or a cast that feels dangerously tight. Urgent review is also needed after a new fall, fever, drainage or a sudden change in the limb’s position.
