Ringworm on the back of the head: what to know

Scalp ringworm can cause itchy scale, tenderness, broken hairs, small areas of hair loss, or swollen patches. It may appear at the back of the head or elsewhere on the scalp. A scalp rash can also be caused by dandruff, psoriasis, bacterial infection, or irritation, so examination is needed before choosing treatment.

The fungus can spread through combs, hats, pillows, towels, and close contact. Children and household members may need checking if they develop scale, itch, or patchy hair loss.

Book a dermatology or primary-care assessment rather than relying on a skin cream. A clinician may also recommend a medicated shampoo to reduce spread. Do not share combs, hats, brushes, towels, or pillowcases.

Wash personal fabrics regularly, clean hair tools, and avoid scratching or picking the area. Do not shave an affected patch without advice, as this can irritate the scalp.

Arrange prompt care for patchy hair loss, broken hairs, pus, crusting, a tender boggy swelling, or swollen glands. Seek urgent help if fever, rapidly increasing swelling, severe pain, or spreading redness develops, because intense inflammation can damage follicles.

Ask whether the scalp needs microscopy or culture, which oral medicine is suitable, whether shampoo can reduce transmission, and who at home should be examined.
