Rheumatoid arthritis and upper back pain

RA is an inflammatory condition that commonly involves the small joints of the hands, wrists, feet, and other areas. Upper back discomfort may come from inflammation around the neck, altered posture, muscle guarding, or a separate muscle, disc, or shoulder problem. Pain that is persistent, worse after rest, or accompanied by joint swelling and morning stiffness deserves assessment. RA symptoms can occur alongside finger stiffness, hand swelling, joint pain, or joint stiffness; these patterns help a clinician decide whether inflammation is active.

Record where the pain sits, when it starts, how long morning stiffness lasts, and whether movement changes it. Arrange a clinical review if symptoms continue or interfere with sleep, work, or movement. A clinician may examine your spine and peripheral joints, review medicines, and decide whether blood tests or imaging are useful. Do not start, stop, or change anti-inflammatory medication without advice, especially if you have stomach, kidney, heart, or blood-pressure concerns.

Seek urgent medical help for upper back pain with new weakness, numbness, trouble walking, loss of bladder or bowel control, fever, severe unrelenting pain, or chest pain and shortness of breath. Contact your rheumatology or primary-care team promptly if a known RA flare brings rapidly increasing pain, swelling, or loss of function.
