Rheumatoid arthritis thumb pain: what to know

Pain may be aching, tender, or sharper when you pinch, grip, twist, or lift. Morning stiffness or discomfort after sitting still can accompany an inflammatory flare. The base of the thumb, the joint in the thumb, and the wrist can each produce pain, and more than one area may be involved. Thumb pain by itself cannot diagnose rheumatoid arthritis because overuse, osteoarthritis, tendon irritation, and injury can feel similar.

Write down when pain appears, whether the hand looks swollen or warm, and how long stiffness lasts after waking. Mention symptoms in the fingers, wrists, or other joints at the same appointment.

Choose tools with broad handles, use both hands for heavier objects, and alternate demanding tasks. Keep the thumb moving gently within a comfortable range, but stop before sharp pain. Cold may soothe a hot swollen area, while warmth can make stiffness easier for some people. A clinician or hand therapist can advise on a splint and exercises suited to your joints.

Continue prescribed disease treatment as directed and ask before adding anti-inflammatory medication. Your clinician should consider stomach, kidney, heart, blood-pressure, pregnancy, and medicine interactions before recommending it.

Get prompt medical help if pain follows a significant injury, the thumb becomes deformed, a joint is hot and swollen with fever, redness spreads quickly, or the hand becomes numb, pale, or blue. Contact your rheumatology or primary-care team if pain is steadily worsening or your usual flare plan is no longer helping.
