Rheumatoid arthritis and pulmonary fibrosis: what to know

Pulmonary fibrosis means that scar tissue can make the lungs stiffer and reduce efficient oxygen movement. Some people have few symptoms early on; others notice a dry cough, breathlessness on exertion or reduced stamina. Breathlessness can also come from heart disease, infection, anaemia, deconditioning or medicine effects, so it should not be attributed to rheumatoid arthritis without assessment.

Evaluation may include listening to your chest, checking oxygen levels, breathing tests and chest imaging. Rheumatology and respiratory teams may work together to distinguish lung inflammation, scarring and other causes.

Tell your clinician about new cough, breathlessness, reduced exercise tolerance, chest discomfort or changes in voice. Avoid smoking and vaping, reduce exposure to dust and fumes, and keep vaccinations up to date according to medical advice. Continue joint treatment as prescribed, but report possible medication effects promptly rather than stopping treatment alone. Pulmonary rehabilitation or adapted exercise may help selected patients.

Arrange prompt review for breathlessness that is new, persistent or worsening, especially with a dry cough or falling stamina. Seek emergency help for severe breathlessness at rest, blue lips, confusion, fainting or sudden chest pain.
