What Should You Know About Rhabdomyolysis Urine Findings?

Rhabdomyolysis happens when damaged muscle releases substances into the bloodstream. Myoglobin, a muscle protein, can pass into urine and make it look brown, red-brown, or unusually dark. A urine dipstick may react as though blood is present even when microscopy finds few or no red blood cells. This pattern can support concern about myoglobin in the urine, but urine colour alone cannot confirm or rule out rhabdomyolysis.

Clinicians usually interpret the urine result alongside symptoms, recent strenuous exercise, injury, prolonged immobility, heat exposure, seizures, or medicines that can damage muscle. Blood tests, especially creatine kinase and kidney-function tests, help assess the extent of muscle injury and whether treatment is needed. Electrolytes may also be checked because muscle breakdown can disturb the body's chemical balance.

Stop strenuous exercise and arrange prompt medical assessment, particularly if dark urine occurs with significant muscle pain, weakness, swelling, or reduced urination. Tell the clinician when the colour change began, what activity or injury preceded it, and which medicines, supplements, or recreational substances you use. Do not restart intense exercise until a clinician has assessed you.

Unless a healthcare professional has told you to restrict fluids for another condition, take fluids normally while arranging care; do not rely on home hydration to treat suspected rhabdomyolysis. A clinician may recommend monitored fluids and repeat blood or urine tests based on your examination and kidney results.

Seek urgent medical care for cola-coloured or clearly bloody urine after intense exertion, an injury, heat exposure, or prolonged pressure on a muscle. Go urgently as well for very little or no urine, worsening weakness, severe muscle swelling or pain, confusion, fainting, chest symptoms, or trouble breathing. These signs can indicate complications that require examination and blood tests rather than observation at home.

Ask whether your urine dipstick and microscopy suggest myoglobin, whether creatine kinase and kidney-function tests are needed, and which symptoms should send you back for urgent review. Also ask when it is safe to return to exercise and whether any medicine or supplement should be paused.
