What the outlook means
Rhabdomyolysis is rapid breakdown of muscle tissue. Contents of damaged muscle enter the bloodstream and can strain the kidneys. Severe cases may also disturb body salts, heart rhythm or fluid balance. The trigger may be intense or unfamiliar exercise, a crush injury, heat illness, prolonged immobility, a seizure, infection, medication or another medical problem.
Clinicians usually consider the amount of muscle damage, urine output, kidney blood tests, body-salt levels, hydration, the underlying trigger and how the person responds to treatment. Dark or tea-coloured urine can occur, but its absence does not rule out rhabdomyolysis. Muscle pain, weakness or swelling may be more pronounced than ordinary post-exercise soreness.
What to do after a possible trigger
Stop strenuous activity and arrange prompt medical assessment if severe muscle pain, marked weakness, swelling or unusually dark urine develops after exercise, heat exposure, injury or prolonged immobility. Do not try to train through these symptoms. A clinician may check blood and urine, monitor kidney function and give carefully selected fluids while treating the cause.
Do not start, stop or change a prescription medicine on your own. Tell the clinician about medicines, supplements, recreational substances, recent illness, exercise intensity, heat exposure and any period when you could not move normally. If you have a fluid restriction or heart or kidney disease, ask before drinking large amounts because fluid advice needs to be individualised.
When to seek urgent care
Seek emergency care for very little or no urine, fainting, confusion, severe weakness, trouble breathing, chest discomfort, a racing or irregular heartbeat, rapidly increasing swelling or severe pain after an injury. These signs can reflect kidney injury, dangerous changes in body salts, major muscle damage or another emergency.
If symptoms are milder but unusual after strenuous exercise, contact a healthcare professional promptly rather than waiting for the soreness to settle. Ordinary muscle soreness after exercise generally improves with rest, while rhabdomyolysis needs testing because symptoms alone cannot measure the degree of muscle breakdown.
