Rhabdomyolysis and renal failure: what to know

When muscle cells are damaged, they release proteins and other cell contents into the bloodstream. The kidneys filter the blood, but a large muscle injury can make this work harder. Kidney trouble may show as much less urine, dark urine, swelling, nausea, unusual tiredness, or confusion. Muscle pain, weakness, and tenderness may occur after intense exercise, a crush injury, prolonged immobility, heat exposure, or certain medicines. Muscle soreness after exercise can have many causes; severe symptoms or urine changes deserve medical attention.

Stop strenuous activity and arrange urgent medical evaluation if rhabdomyolysis is suspected. Do not try to flush it out by forcing large amounts of water, especially if you have heart or kidney problems. A clinician may check muscle enzymes, kidney function, salts in the blood, urine, and your medicines or recent exertion. Treatment can include monitored fluids and correction of chemical imbalances; severe kidney problems may require hospital-based support.

Go to an emergency service for very little or no urine, cola-coloured urine, worsening weakness, fainting, confusion, breathing difficulty, chest discomfort, or severe swelling. Seek prompt care after a major injury or prolonged compression, even if pain seems manageable. Kidney injury can develop while muscle symptoms are changing, so waiting for symptoms to settle is unsafe.

Ask which results show muscle breakdown, whether kidney function needs repeat testing, how much activity and fluid are appropriate, and which medicines or supplements should be paused. Clarify what urine changes or symptoms should trigger immediate return.
