What should you know about removing an intramedullary nail?

An intramedullary nail sits inside the hollow centre of a long bone to help hold a fracture while it heals. Removal is not automatic. An orthopaedic surgeon first checks healing, symptoms, the nail's position and the original injury. Prominent locking screws, local irritation, infection or another implant-related concern may change the balance. Removing hardware before the bone is ready can leave it less supported.

The operation uses the previous access points when practical, although scar tissue and buried or difficult screws can affect the plan. The surgeon will explain anaesthesia, wound care, weight-bearing and whether a period of protection is needed.

Bring a list of medicines and tell the surgical team about allergies, smoking or health problems that may affect healing. Arrange transport and help for the first part of recovery. Follow the exact instructions for walking, crutches, dressing care and exercises. Keep wounds clean and avoid soaking them until advised. Increase activity gradually; pain or swelling after a step up means the plan may need review.

Contact the surgical team for increasing wound redness, drainage, fever, worsening pain or a wound that opens. Seek urgent help for severe calf swelling or pain, chest pain, breathlessness, a cold or numb foot, or sudden inability to bear weight. A fall or new deformity also needs prompt assessment.

Ask whether the fracture is fully healed, what benefit removal is expected to provide, which parts will be removed, and what protection follows surgery. Clarify driving, work, exercise, wound review and signs that require urgent contact.
