Red rash on lower legs that is not itchy: what to know

Redness without itch can follow irritation, pressure, inflammation, a medicine reaction, or changes affecting small blood vessels. Note whether the marks fade when pressed, feel raised, look like pinpoint dots, form plaques, or sit around swollen ankles. Scaly patches may fit psoriasis or ringworm, while heat and rubbing can contribute to a rash. A non-itchy appearance does not identify the cause, and a clinician may need to examine the skin closely and review medicines and recent illness.

Take clear photographs in consistent light and mark the edge of spreading redness with a pen if appropriate. Avoid new creams, vigorous rubbing, hot showers, and tight socks until the pattern is clearer. Wash gently, moisturise dry skin, and keep the legs protected from sun and friction. Do not start leftover steroid or antibiotic treatment without advice. Note pain, warmth, swelling, bruising, fever, breathlessness, and whether the spots fade with pressure.

Seek urgent care for purple or bruise-like spots that do not fade with pressure, rapidly spreading redness, severe pain, one-sided leg swelling, chest pain, breathlessness, faintness, or fever with marked illness. Prompt review is also needed for warmth, tenderness, pus, red streaks, new bleeding, or a rash beginning soon after a medicine. Make an appointment for a persistent or recurrent rash, even if it remains comfortable.

Ask whether the colour and texture suggest irritation, psoriasis, ringworm, a medicine reaction, or another process; whether pressure-fading matters; and whether any examination or blood test is needed.
