Red rash on body and fever: what to know

A rash may be flat, raised, blistered, scaly, or tender. Fever can occur with viral illnesses such as chickenpox or COVID-19, and with bacterial skin infection such as cellulitis. Chickenpox often causes itchy spots that become fluid-filled blisters, while cellulitis tends to produce a warm, painful, spreading area. A rash with fever does not identify one diagnosis by itself. The appearance, speed of spread, recent exposures, medicines, and other symptoms help a clinician assess it.

Record when the fever and rash began, where the rash appeared first, and whether it is spreading. Check the skin in good light without scratching it, drink fluids, and rest. Avoid sharing towels, clothing, or close contact if an infectious illness is possible. Do not apply a new cream or take leftover antibiotics without advice. A clinician may need to examine the rash, especially when it is painful, rapidly changing, or affecting a child, older adult, or person with reduced immunity.

Seek urgent medical help for trouble breathing, confusion, fainting, severe drowsiness, a stiff neck, severe headache, or a rash that does not fade briefly when pressed. Get prompt assessment if the skin becomes very painful, hot, swollen, or rapidly spreads, or if there are blisters around the eyes or mouth. Arrange medical advice for a persistent fever, worsening symptoms, or concern about contagious illness.

Ask which features point toward a viral illness, bacterial skin infection, or another cause; whether testing is needed; how to protect household contacts; and which changes should trigger urgent review.
