Prostate cancer recurrence after brachytherapy: what to know

How the structures involved in Prostate Cancer differ from normal

Short answer

Recurrence of prostate cancer after brachytherapy is assessed through PSA trends, examination, imaging, and sometimes biopsy rather than one result alone.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How doctors interpret a possible recurrence

Brachytherapy places radioactive sources in or near the prostate to treat cancer. PSA can change during recovery, and a single result may not explain what is happening. The team reviews the pattern over time alongside urinary, bowel, sexual, and general symptoms. A rise may prompt repeat PSA, examination, MRI, or other imaging; biopsy is considered when tissue confirmation would guide care. Recurrence can be local, outside the prostate, or involve distant sites, and the assessment is designed to locate it.

What to do after a concerning PSA or symptom

Keep every PSA result and attend the follow-up schedule recommended by your oncology or urology team. Report new difficulty passing urine, blood in urine or stool, persistent bone pain, unexplained weakness, or worsening pelvic symptoms. Ask which medicines or urinary procedures could affect interpretation. Treatment depends on where cancer is found, previous radiation, overall health, and goals. Options may include further radiotherapy, prostate removal surgery, hormone-based or targeted radioisotope therapy, or supportive bone-strengthening therapy.

When urgent help is needed

Seek urgent care if you cannot pass urine, have severe lower abdominal pain, heavy bleeding, fever with urinary symptoms, new leg weakness, or loss of bladder or bowel control. These signs can reflect urinary blockage, infection, bleeding, or pressure on nerves and need prompt assessment. Do not wait for the next PSA appointment when symptoms are sudden or rapidly worsening.

Questions for your doctor

What does my PSA pattern show over time? Could treatment effects or another condition explain the change? Which scan or biopsy would clarify the location? Is the concern local or elsewhere? How would each treatment affect urinary, bowel, and sexual function?

Questions people ask

Does one rising PSA prove recurrence?

No. Doctors interpret PSA over time and alongside examination, treatment history, imaging, and sometimes biopsy.

Can urinary difficulty happen after brachytherapy without recurrence?

Yes. Treatment effects and other urinary conditions can cause difficulty passing urine, but new or worsening symptoms should be assessed.
