Rectal pain in the middle of the night

Brief, sharp pain that arrives suddenly and settles may reflect spasm of the muscles around the anus. Longer-lasting pain can occur with constipation, a fissure, haemorrhoids, infection or inflammation. The location described as rectal may also be referred from the pelvis, lower back or nearby tissues, so the pattern alone cannot identify a cause.

A clinician will ask about bowel movements, bleeding, fever, discharge, urinary symptoms, abdominal pain, sexual health and whether the pain is new or recurring. An examination may be needed even when the pain disappears between episodes.

Note when episodes begin, how long they last and whether bowel movements trigger them. Avoid straining, keep fluids and fibre appropriate for you, and use gentle warmth if it is comfortable. Do not insert products or take repeated medicines without checking that they are suitable, particularly if you have bleeding, pregnancy, kidney disease or other medical conditions.

Seek urgent help for heavy or persistent bleeding, black stools, fever, pus, a painful lump, severe or worsening pain, vomiting, a swollen abdomen, fainting, or inability to pass stool or urine. New night pain that repeatedly wakes you, or pain with unexplained weight loss, should be assessed promptly.

Ask what examination is needed, whether constipation or a fissure could explain the pattern, and which warning signs require emergency care. Share medicines, bowel changes, bleeding and any recent procedure or injury.
