Rashes on the body with fever: what to know

Viral illnesses can cause fever with widespread spots, bumps, or a blotchy rash. Chickenpox often produces itchy spots that develop into fluid-filled blisters in crops. A hot, painful, expanding patch may be cellulitis. Fever with cough, breathing symptoms, stomach upset, sore throat, or recent travel adds useful clues; it does not establish a diagnosis by itself. A new rash after a medicine may represent a drug reaction and should be discussed promptly.

Rest, drink fluids, wear light clothing, and avoid sharing towels or close contact if an infectious illness is possible. Record when the fever and rash began, where the rash started, and whether it is itchy, painful, blistered, or changing. Do not give medicines intended for another person, and check dosing advice carefully for children. Contact a clinician the same day for a new widespread rash with fever, particularly in a young child, older adult, pregnant person, or anyone with reduced immunity.

Call emergency services for breathing trouble, facial swelling, collapse, confusion, severe drowsiness, a stiff neck, a severe headache, seizures, or a purple rash that does not fade when pressed. Urgent assessment is also needed for rapidly worsening illness, skin blistering or peeling, severe pain, dehydration, or involvement of the eyes or mouth. Keep the person away from vulnerable contacts until a clinician advises whether the illness may spread.

Ask whether examination or testing is needed, whether the illness could spread to others, how to manage fever safely, and which symptoms mean you should return immediately. Mention every recent medicine, vaccine, travel exposure, and known contact with illness.
