Rashes in skin medicine: how clinicians assess them

A rash may involve redness, scaling, bumps, blisters, crusting, or changes in texture. Atopic Dermatitis often affects dry, itchy skin; Contact Dermatitis follows exposure to an irritant or allergen; and Heat Rash is linked with trapped sweat. Ringworm may form a spreading ring, while Scabies can cause intense itch and affect close contacts. Psoriasis, Intertrigo, and Drug Rash have distinct clues, but overlapping appearances mean a photograph or brief description cannot always identify the cause.

Note when the rash began, where it started, how it changed, and whether itch or pain is present. List new medicines, foods, cosmetics, detergents, plants, travel, heat exposure, and sick contacts. Use gentle cleansing, avoid picking, and pause newly introduced fragranced products when practical. Do not apply several medicated creams at once, because this can alter the appearance and irritate the skin.

Emergency care is needed for breathing trouble, facial swelling, faintness, extensive blistering, skin peeling, or sores in the eyes or mouth. Seek prompt assessment for fever, severe pain, pus, rapidly spreading redness, or a rash after a new medicine. Make a routine appointment for a persistent, recurrent, spreading, or sleep-disrupting rash.
