What the scan can show

A quadriceps tendon ultrasound uses a handheld probe moved over gel on the skin above the kneecap. It creates real-time images of the tendon and can compare the injured side with the other knee. The examiner may view the tendon while the knee is gently positioned or the thigh muscles are lightly activated, if this is safe.

The result may describe tendon thickening, a split within the fibres, a gap between torn ends, fluid, or nearby soft-tissue swelling. Ultrasound is different from an X-ray, which mainly shows bone. If the findings do not match the examination, another scan or specialist review may be considered.

Preparing and following up

Wear clothing that allows access to the thigh and knee, and tell the clinician when the injury happened and what movement caused it. The scan itself usually involves skin contact and repositioning rather than an injection. Keep following any brace or weight-bearing instructions while waiting for the report.

A scan result is interpreted alongside strength, swelling, the ability to raise a straight leg, and your daily needs. A partial injury may be managed with protection and rehabilitation, while a complete separation may require discussion of tendon repair surgery. Rehabilitation is then adjusted as healing progresses.

When to seek care

Do not wait for a routine scan if you suddenly cannot straighten the knee, cannot lift the straight leg, or have rapidly increasing swelling after an injury. Seek urgent help for a deformed knee, numbness or unusual coldness in the foot, severe uncontrolled pain, or an open wound. Imaging should support assessment, not delay urgent protection of the leg.

Useful questions

Ask what the ultrasound shows about tendon continuity, whether the tear is partial or complete, and whether another form of imaging is needed. Clarify how to use a brace, how much weight to place on the leg, and when post-operative rehabilitation or other supervised exercise would be appropriate.
