What should you know about pure red cell aplasia with thymoma?

The bone marrow normally makes red blood cells, which carry oxygen. In pure red cell aplasia, the marrow makes very few or no new red cell precursors, while white blood cells and platelets may be less affected. This can cause anaemia with tiredness, breathlessness on exertion, dizziness, headache or a fast heartbeat.

A thymoma is a tumour arising in the thymus, an organ behind the breastbone. In some people, immune activity linked with a thymoma interferes with red blood cell production. The association does not explain every case of anaemia, so clinicians use blood counts, a reticulocyte count, examination and sometimes bone-marrow testing to confirm the pattern and look for other causes.

Care usually involves haematology and thoracic or oncology specialists. The team may discuss tumour removal surgery when it is appropriate, alongside treatment directed at the abnormal immune response and supportive care for anaemia. Radiotherapy may be considered for the thymoma depending on its features and the operation findings.

Bring a list of medicines, supplements and previous blood results. Ask how often counts will be checked, whether activity should be limited while anaemia is severe, and what symptoms mean that a transfusion or urgent review may be needed. Do not start iron or other supplements for this condition without advice, because the problem is reduced red-cell production rather than simply low dietary iron.

Seek urgent care for chest pain, breathlessness at rest, fainting, new confusion, a very fast heartbeat or sudden marked weakness. Contact the clinical team promptly for worsening fatigue, fever, unusual bleeding, black stools or new swallowing or breathing difficulty, especially while treatment is being planned.
