PSMA PET/CT for Prostate Cancer: What to Know

How the structures involved in Prostate Cancer differ from normal

Short answer

PSMA PET/CT is an imaging scan that can help locate prostate cancer cells and guide treatment decisions.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Scan type
PET imaging combined with CT
Tracer
A PSMA-targeting radioactive tracer
Main use
Locating and assessing prostate cancer

The short answer

It combines a PET scan with CT imaging. A small amount of a radioactive tracer that targets prostate-specific membrane antigen, or PSMA, is given through a vein. Areas that take up the tracer can appear on the images, while CT adds detail about the body's structure. The scan may help assess whether cancer is confined to the prostate, has reached lymph nodes or bones, or has returned after treatment.

What the scan can mean

A brighter area on a PSMA image does not automatically prove cancer. Some non-cancerous conditions and other tissues can also show tracer activity, and very small or PSMA-poor cancer deposits may be less visible. A specialist interprets the images alongside your PSA trend, biopsy findings, examination, and previous scans.

The report may describe uptake, lymph nodes, bone lesions, or organs separately. It may also compare the scan with earlier imaging. The result can change whether your team discusses surgery, radiotherapy, systemic treatment, or a targeted radioisotope approach. It does not by itself select a treatment or predict exactly how an individual will respond.

How to prepare and what to expect

Tell the imaging team about medicines, allergies, kidney problems, diabetes, recent procedures, and any possibility of pregnancy or breastfeeding. Follow the centre's instructions about eating, drinking, and arrival time; preparation can differ depending on the tracer and your health.

After the tracer is administered, you usually rest quietly while it circulates, then lie still for imaging. The appointment can take several hours because it includes preparation, an uptake period, and scanning. Drinking fluids and urinating as instructed can help clear the tracer. Ask when normal activities, work, driving, and contact with young children or pregnant people are appropriate for your specific scan.

When to seek care

Contact the imaging or oncology team promptly if you develop a rash, swelling, breathing difficulty, faintness, or other sudden symptoms after the injection. Seek urgent medical help for severe breathing trouble or collapse.

A scan appointment should not delay help for an emergency. Seek urgent care if you cannot pass urine, have new severe back pain with leg weakness or numbness, or lose control of your bladder or bowel. These symptoms need assessment because prostate cancer or another condition can affect the urinary tract or nerves.

Questions for your doctor

Ask why PSMA PET/CT is recommended, what the team is looking for, whether another scan is needed, and when the report will be discussed. You can also ask how the findings might affect surgery, radiotherapy, hormone treatment, or targeted radioisotope therapy; whether a biopsy or follow-up scan is needed; and whom to contact about symptoms after the appointment.

Questions people ask

Does a PSMA PET/CT scan diagnose prostate cancer on its own?

No. A specialist interprets the scan with your clinical history, PSA results, biopsy information, and other imaging. The scan can support diagnosis and staging decisions but is not the only source of evidence.

Can PSMA PET/CT show cancer that has returned?

It may help locate recurrent prostate cancer, including disease in lymph nodes or bones, when your team considers the scan appropriate. The meaning depends on the full clinical picture and the scan findings.

What happens after the scan?

Your care team reviews the report with you and explains whether it changes the next step. That may include monitoring, additional testing, surgery, radiotherapy, or medicine-based treatment.
