What should you know about prostatitis causes?

Short answer
Prostatitis can result from a bacterial infection or from pelvic inflammation without a confirmed infection.

Why prostatitis happens
Bacteria can reach the prostate through the urinary tract and cause inflammation. This may occur alongside a urinary tract infection and can produce painful urination, urgency, frequent urination, pelvic pain, fever, or difficulty passing urine.
Some people have ongoing prostate or pelvic pain without bacteria being found. Muscle tension in the pelvic floor, irritation of nearby nerves, and changes in how pain signals are processed may contribute. A cause is not always identifiable, and symptoms do not by themselves show which type is present.
How causes are assessed
A GP consultation can cover the timing of symptoms, urinary flow, sexual symptoms, bowel symptoms, fever, previous infections, and medicines. Urine tests and an examination may help identify infection or another urinary problem. Treatment is selected for the findings: antibiotics may be used when bacterial infection is suspected, while persistent non-infectious pain may need a different plan.
When to seek care
Arrange prompt care for new pelvic pain with urinary symptoms, especially when symptoms are worsening. Seek urgent help if you cannot pass urine, have fever with shaking chills, become confused or faint, or feel severely unwell. These features can signal an acute infection or urinary obstruction.
Questions for your doctor
What findings suggest infection in my case? Do I need a urine test or examination before antibiotics? Could pelvic-floor tension or another urinary condition contribute, and what should I do if symptoms continue after the first treatment plan?
