Prostate cancer and vertebral metastases: what to know

Cancer in a vertebra can weaken the bone or irritate nearby tissues. Pain may be persistent, worse at night or aggravated by movement. If a growing area presses on the spinal cord or nerve roots, numbness, tingling, weakness or changes in walking may develop. Bladder or bowel control can also be affected. Imaging helps the team assess the spine and plan care.

Treatment aims may include controlling cancer, easing pain, protecting spinal stability and preventing or relieving pressure on nerves. Options can include radiotherapy, medicines that support bone strength, systemic cancer treatment or surgery in selected situations.

Tell your oncology team about the location, severity and pattern of pain, especially if it is new or worsening. Ask whether you need activity or lifting limits and which pain medicines are suitable. Use walking aids if advised and avoid changing activity suddenly when the spine may be fragile. Keep appointments for imaging and treatment planning, because symptoms and scan findings are reviewed together.

Seek emergency medical help for new leg weakness, numbness around the groin, loss of bladder or bowel control, inability to walk, or severe rapidly worsening back pain. These can signal spinal cord or nerve compression. Do not wait for a routine appointment; tell emergency staff that prostate cancer may involve the spine.

Ask which vertebrae are affected, whether the spinal cord or nerves are under pressure, what activity is safe, and how pain will be managed. Ask how radiotherapy, bone-strengthening treatment, medicines or surgery fit together, and which symptoms should trigger immediate help.
