Prostate cancer spread to lymph nodes: what to know

Lymph nodes are small immune-system structures that drain fluid from nearby tissues. Prostate cancer can reach pelvic nodes and, in some cases, nodes farther from the prostate. Enlarged nodes may cause no symptoms; location and size can occasionally contribute to leg swelling, abdominal discomfort or urinary problems. A swollen node is not automatically cancer, because infection and inflammation can enlarge nodes too.

Clinicians use examination, imaging, blood tests and sometimes tissue sampling to assess what is happening. Node involvement is interpreted alongside prostate findings, bone scans or other imaging, symptoms and treatment history.

Bring scan reports and medication details to oncology visits, and mention swelling, pain or changes in urination. Depending on the distribution and treatment goal, care may involve hormone-based treatment, radiotherapy, surgery in selected situations or other systemic treatment. Your team may monitor with examinations, blood tests and repeat imaging. Ask about support for urinary, bowel, sexual and emotional effects.

Seek urgent help for inability to pass urine, severe abdominal or back pain, new leg weakness, loss of bladder or bowel control, or rapidly increasing leg swelling with breathlessness. Contact your cancer team promptly for persistent urinary changes, growing swelling, fever or pain. These symptoms can have causes other than cancer but should not be ignored.

Ask which lymph nodes are involved, whether the findings are confirmed, whether other sites need checking, and what the treatment goal is. Ask how response will be measured, which urinary symptoms matter, and when to call between appointments.
