Prostate cancer spread to the hip: what to know

Cancer in the hip may involve the pelvis, the upper thigh bone or nearby tissues. Pain can be felt in the groin, buttock, thigh or lower back and may worsen with standing or walking. A scan is needed to distinguish cancer involvement from arthritis, muscle strain, fracture or another cause. The team also checks whether the bone is weakened and whether the hip joint is at risk.

A hip lesion does not determine the whole outlook by itself. Doctors consider the wider pattern of prostate cancer, symptoms, physical function and response to treatment.

Tell your oncology team about pain that is new, persistent or changing, especially if it limits weight-bearing. Imaging may guide radiotherapy to a painful area, medicine for the cancer, bone-strengthening therapy, pain relief or an orthopedic review. Use a walking aid only with suitable instruction, and ask how much weight-bearing is safe. Avoid falls and ask before lifting or high-impact activity.

Seek urgent care for sudden severe hip or thigh pain, a new inability to bear weight, a shortened or turned-out leg, or a fall followed by pain. Contact the cancer team promptly for increasing pain, new weakness or difficulty walking. New bladder or bowel problems with back pain need emergency assessment.

Ask which bone is involved, whether it is structurally weakened, what activity is safe, and whether an orthopedic or radiation specialist should review you. Ask how pain will be measured and when follow-up imaging is planned.
